The epidural test needle dose consists of a small amount of local anesthetic. The rationale is that injection of such a small amount into the intrathecal space produces an easily identifiable motor and sensory cord block without producing an unacceptably high level of spinal anesthesia. If the same tested dose is injected epidurally, it should have minimal or no apparent effect. A typical test dose might be 40 to 60 mg of lidocaine, which if given intrathecally produces relatively low levels of signs and symptoms of spinal cord block very quickly. It must also be remembered that if a combined spinal and epidural anesthesia is administered and the patient receives adequate spinal anesthesia for either high or low level of surgical anesthesia, any subsequent epidural trial needle dose may result in a high level of spinal anesthesia.
